All students

• Emergency Information Form*
• Dismissal Authorization Form*
• White Medical Form (completed and signed by a pediatrician)*

Returning students only

• Personal and Family History Update**

New students only

• Personal and Family History*
• Yellow Medical Form*
• Permission and Release Form*
• Language Selection Form**

* If you cannot print these forms please do not hesitate to contact the school for paper copies.

** If you have difficulty filling out any of these forms online please ask for help in the office.

Early Drop-Off Sign Up (for morning students only)**

Students in the 2s and 2/3s classes should bring these items marked clearly with their names:

• 2 bags of diapers
• 2 bags of wipes
• a sippy cup (if he/she uses one)
• additional clothing if bathroom accidents still occur

Stduents in the 3s, 3/4s and 4s classes should bring the following items marked cleary with their names:

• diapers if they are still used
• additional clothing if bathroom accidents still occur
• sunscreen
• a hat
